Faculty or staff who do not have a Blackboard account should fill out this form.
Be sure to fill in your CSUFRESNO email address. If you do not have a course that is footnoted as either '15' or '17' in PeopleSoft, but need a development course shell, be sure to fill out the Development Course Request Form when you are done requesting your Blackboard account. Please fill out the entire form and click "Submit" when done.
